Death swap

1. Fixed a bug where it didn't swap the players when "exclude non-survival players" setting was off

2. Added a new setting "Only swap at 5-minute mark (consistent swapping)": if enabled swaps will not be random between 3 and 5 minutes, and will always happen when minute timer hits 5.

3. Added symbols next to the timer for more clarity of swapping consistency:

• 🕘: Consistent swapping (safe until 5-minute mark)

While inconsistent swapping:

• ✔: no risk of swapping (safe)
• ❗: low risk of swapping
• ❗❗: medium risk of swapping
• ❗❗❗: high risk of swapping
• 🔄: swapping
