When a player dies they leave a grave which contain their items. Settings are highly configurable

Updated to 1.21.3
- Falling into the void in the end causes the grave to spawn on a nearby island
- Added config option Void Recovery to disable this feature and control how close the island must be
- Updated tags/item/rare_items to match reworked item rarities
- Minor optimization for when dispawn time limits are enabled
