Added disable-load-balancer label to disable load balancer (as the name says...)

The backing Pods are still registered normally. When used with disable-name-suffix, you can completely get rid of -1 suffix in server name.

• Fix race condition of disable-name-suffix occurs when pod restarts

• Allow specifying the server name via annotation by Phoenix616
• Add ability to configure the prefix of the label keys by Phoenix616
• Add disable-name-suffix label to register the server name exactly as preferred-server-name (no -1 suffix)
