Update chunk effects and remove monster/timer Features
- Remove surprise monster spawning and related /rc spawnmonster commands
- Remove action bar timer and timer/reset commands
- Set /rc effects default to false
- Make chunk effects non-stacking and clear them on chunk changes, stop, disable, or Chunk Walker protection
- Keep start/stop, fluids, chunk replacement, blacklist, debug, and Chunk Walker behavior
- Bump mod version to 2.2.0
- Update README with current commands and /rc start requirement
