Access Denied v6.4.1 (NeoForge) & Access Denied v6.2.1 (Fabric) for Minecraft 1.21.1

This is a minor hotfix/patch addressing a game-breaking bug and a couple of oversights. Existing configs are fine.

It is highly recommended to update to this version.

• Fixed mod crashing server side due to improper checking for config directories.
• Added matteo335 to the credits. (Thank you for reporting the config issue.)
• Lowered hard fabric loader requirement from >=0.18.3 to >=0.16.0.
• Re-added logging when a player tries to join a denied dimension.

Unless there are any more issues this should be the last update until v7.0.0.
