🐛 Critical Fix

Fixed Offense Counting Bug

Issue: Players were incorrectly getting "2nd offense" when they should have gotten "1st offense", even though another player from a different IP was banned.

Fix: The offense counting now properly tracks by IP address or player UUID based on the template's track_by_ip setting:

• IP-based tracking: All players from the same IP share the same offense count
• UUID-based tracking: Each player has their own separate offense count
• Result: Escalation levels now correctly apply per player/IP group

Impact: Ban durations now correctly reflect the actual offense count for each player or IP address.

🚀 Database Improvements

Auto-Increment ID Support

• All database tables now have AUTO_INCREMENT IDs
• Works for MySQL, MariaDB, H2, and SQLite
• Automatic migration for existing databases

🔧 Technical Details

• Fixed offense counting to use proper WHERE clauses
• Now filters by target_ip or target_uuid based on tracking key
• Properly tracks escalation levels per player/IP group
