🆕 New Placeholder — Accurate Online Count

• Added %vanishium_online_count% — the real number of online players with vanished players excluded. A drop-in 100% accurate player counter for tab lists, scoreboards and MOTDs, with no math placeholders or workarounds needed.

🐛 Fixes

• Vanished count no longer counts offline players. A player who logged out while vanished was still being counted by %vanishium_vanished_count%. The in-memory vanish set is now cleared on quit, so vanished_count (and the new online_count) only ever reflect players who are actually online. Their vanish state is still saved and restored on rejoin / across servers.
