Resurrections
This is a Minecraft mod I made for my hardcore multiplayer server. After a player dies, they drop their soul which can be placed in the reviving station and revived.

Souls sometimes disappearing when the Resurrecting Station is destroyed, unloaded, or is changed.
Hologram player still existing after supposedly being removed.
