Respawn Menu

This version adds UI and functionality:

• Pressing R opens a menu in which you can choose to respawn.
• It works in all gamemodes, even if cheats are off.
• When dying, you die normally, i.e. all of your items drop; you are sent to the death screen; and return to world spawn.
• Respawning works on public servers, if the server has it installed.
