v1.4.0 Fixed several bugs.

Fixed wrong key type: FTCLeakFixer.java:130 now uses getStringUUID().

Fixed client-side firing: onEntityLeave calls getLevel()

Fixed init race: init() is private static synchronized in both FTCLeakFixer and FTCConfigReader, with the flag set at the end. Also switched to Class.forName(name, false, loader) so FTC's static initializers no longer run on whatever thread fires the event, and both now catch LinkageError alongside ClassNotFoundException.

Added missing maps: added windowCooldowns, torchCooldowns, noteBlockCooldowns, trapdoorCooldowns, chestCooldowns, lockedChests, jumpscareReadyChests on block-break, and activeJumpscares on logout.
