Reliability + Discord Webhooks Update

This release combines the recent Smart Reconnect Reliability patch with new Discord outbound webhook notifications.

Smart reconnect reliability

• Added disconnect-reason filtering for non-recoverable disconnect text.
• Added max consecutive reconnect attempt cap.
• Added incremental reconnect backoff.
• Added disconnect-screen Reconnect Anyway manual override.
• Added reliability state tracking and reset behavior.

Discord outbound webhooks

• Added global Discord webhook notifications (outbound POST only).
• Supported webhook hosts:

  • ptb.discord.com
  • canary.discord.com
• Added notifications for:
  • disconnect
  • reconnect triggered / reconnect success
  • reliability blocked
  • manual override / manual stop
• Added event toggles, embed/plain format toggle, and privacy toggles.
• Added payload truncation limits to reduce 400 errors.
• Added retry behavior with rate-limit header handling (Retry-After, X-RateLimit-Reset-After).
• Added per-disconnect-screen dedupe for disconnect and reliability-blocked events.

Important constraints

• Discord support is outbound webhooks only.
• No bot token, no slash commands, no inbound Discord control.
• Existing ntfy remote control behavior is unchanged.
