Phase's Discord Rich Presence

Another update at long last! Featuring a lot of Multiplayer changes! (and 100k downloads soon?)

• Instead of showing Server IP, mod will now show Server Name as you defined in the Server List (Or "Minecraft Server" if you are using direct connection) #38
• Added ability to show Server Icon in Presence in both Simple and Advanced Mode #39
• Added separate image key selection for Singleplayer and Multiplayer in Advanced Mode
• Added 6 new images to Advanced Mode
• (26.1 ONLY) Migrated mappings for future releases. Will eventually do this for other version, but for this first release, mappings change only on 26.1!
• Dropped support for 1.21.6-1.21.8
