Revivalix
Adds a simple revival system where death is permanent until another player revives the deceased using a time-consuming recipe or other cruel methods.

Changes:
- Datapack now uses a scoreboard to track whether a player is alive or not instead of teams (looks cleaner and allows for other datapacks to use teams)
- Made the exclusion time actionbar less wide
- Starting the exclusion time now executes a /playsound and a /tellraw
- Removed an unused function from the 1.21+ datapack
Bug Fixes:
- Players who joined during exclusion time and left before it ended no longer stay in that excluded state after rejoining
